Job Description
Radius Licensed Practical Nurses are a vital component of the Team Based care at Radius. Clinical teams of nurses, physicians, nurse practitioners, medical office assistants, outreach workers, health advocates, mental health therapists, and other professionals offer comprehensive, wrap-around care to Edmonton’s most marginalized population. Nursing staff are required to work to full-scope of practice within a primary health care setting to serve all aspects of client needs. LPNs are leaders and independent practitioners at Radius, working collaboratively to ensure high quality care, continuity of health services, and all possible social and medical supports for clients.
This role will be based primarily offsite at the Navigation Centre. The purpose of the Navigation Centre is to connect people living in encampments with a number of crucial services in one location.
Key Responsibilities
- Work in an interdisciplinary team to provide medical supports in a primary care setting
- In collaboration with the healthcare team, providing the following healthcare services:
- Assessment and triage
- Basic treatment of injuries and ailments (including wound care)
- Laboratory collection
- Medication management (including medication administration and medication assistance)
- Management of substance misuse disorders
- Continuity of care
- Client advocacy
- Be an essential part of clinic workflow, including managing patients through all aspects of their interactions at Radius, managing appointments and information within the Electronic Medical Record, and providing direct and indirect supports through the clinic space
Qualifications
- Current registration with CLPNA
- Current CPR
- Mandatory vulnerable sector check and suitable criminal record check
- Experience with marginalized populations (e.g. homeless serving sector, social service agencies) an asset
- *applications from paramedics will also be considered
Knowledge/Skills
- Able to balance numerous demands and conflicting priorities, strong problem-solving skills.
- Ability to work co-operatively within a multidisciplinary team
- Ability to demonstrate patience and understanding for those with addictions and behavioral disorders
- Strong computer proficiency, with experience with MedAccess EMR an asset
- Excellent communication skills.
- Has a good understanding of trauma-informed care, and must be able to operate within a harm reduction model of care
- Excellent understanding of cultural awareness an asset
