Job Summary

The nurse in this position works as part of a multidisciplinary team and is responsible for coordinating and providing assessment, planning, implementation, and evaluation services for clients who have, or are at risk, for sexually transmitted or blood borne infections (STBBI). The nurse will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of clients. Duties include proper assessments, screening, diagnosis, treatment, and client education. This position involves working in a variety of settings, including Radius’ main clinic and other program sites, as well as community outreach locations. During community outreach clients will be engaged wherever those in need of services may congregate (community drop-in centres, shelters, etc.).

Key Responsibilities

•    Work in an interdisciplinary team to provide medical supports in a community outreach capacity

•    Health assessment, screening, diagnosis, and treatment of STBBI’s within LPN scope of practice

•    Provides education on harm reduction practices and disease prevention while promoting healthy lifestyle choices within the context of individuals’ health needs and practices.

•    Collect relevant statistical data to maintain/provide accurate monthly reports

•    General nursing duties

Qualifications 

•    Completion of LPN Diploma

•    Active registration in good standing with CLPNA

•    Experience working with marginalized populations.

•    Minimum two years nursing experience, preferably in primary care

•    Acceptable criminal record and vulnerable sector check within the last year

•    Current CPR certification

•    Experience in phlebotomy and swab collection an asset

•    Recent experience in the assessment, diagnosis, and treatment of STBBI’s an asset

Knowledge/Skills

•    Able to balance numerous demands and conflicting priorities, strong problem-solving skills.

•    Ability to work co-operatively within a multidisciplinary team

•    Experience with an Electronic Medical Record

•    Demonstrate flexibility in organizing work and managing caseloads

•    Excellent verbal and written communication skills

•    Demonstrate initiative, good judgment, and resourcefulness

•    Non-judgmental and respectful of various individual, family and community lifestyles

•    Have a good understanding of trauma-informed care, and must be able to operate within a harm reduction model of care
