About Radius
Radius provides interdisciplinary, team based, integrated, comprehensive, and person-centered primary health care for vulnerable Edmontonians with complex needs. Through primary care and health promotion, Radius improves the quality of life and well-being of populations and individuals who face multiple barriers to accessing health services. Radius achieves this by providing the highest standard of evidence-informed and client focused inter-professional care. We collaborate with our clients to address the broad determinants of health.
Radius operates with a Community Health Centre model, which dictates that we serve a targeted clientele with full-scope, wrap-around services. Our clinic’s target population includes individuals who may be experiencing poverty, mental health conditions, substance misuse, and involvement with the criminal justice system. Our primary care providers, comprised of nurse practitioners and physicians, are supported by a robust team of nurses, medical office assistants, health advocates, mental health therapists, and outreach workers.
Job description
Nurse Practitioners at BMHC are expected to practice full-scope care in a variety of health care
settings. With the support of our interdisciplinary team and collaboration and mentorship from fellow NPs and physicians, nurse practitioners may be called upon to
- Be the primary care provider for an independent patient panel
- Be able to provide scheduled, opportunistic, and outreach care in a variety of care settings, which include our main clinic, offsite clinic locations, and our mobile health clinic
- Diagnose diseases, disorders, and conditions, and provide treatment and ongoing care of chronic and acute medical conditions
- Work within the healthcare system to facilitate consultation, collaboration, and referrals to specialists, other healthcare professionals, other health and social service providers,
and any other applicable disciplines to achieve patient outcomes
Key Responsibilities
Nurse Practitioners at BMHC are expected to practice full-scope care in a variety of health care
settings. With the support of our interdisciplinary team and collaboration and mentorship from fellow NPs and physicians, nurse practitioners may be called upon to
- Provide full-scope primary care, including utilizing appropriate diagnostic, laboratory, and screening tools, and be comfortable with treatment modalities
including pharmacological, complimentary, and counselling therapies to
support complex health needs - Be familiar and comfortable with principles of addictions medicine, including opioidagonist therapies and harm reduction principles
- Keep complete, accurate, and timely documentation of all patient interaction
